Illinois, Iowa, Indiana, Minnesota, Michigan, Missouri, Ohio, and Wisconsin.

In the future, we will be collecting sales tax for additional states. If you have a completed sales tax exemption form, please submit your information to Fertilizer Dealer Supply via email to info@fertilizerdealer.com. If no tax exemption form has been completed, please find your state below for a PDF to fill out and return to us for our records. Any changes to your exempt information must be updated immediately. Sales tax exemption forms must be renewed every three years.
